We offer My Health Online, a web based system where patients can make routine appointments and order repeat prescriptions. Please ask reception for additional details about this service.

Practice Area

The Practice covers the top end of Beaufort to bottom end of Cwm.

We are NOT a training practice.

Patients Preference of Practitioner

Any patient registered with the Practice may express in writing their preference to see a particular GP. Where possible, we will endeavor to accommodate patient’s wishes.

 

New Patients

The Practice welcomes new patients. You can register by either bringing along your medical card or completing a registration form available from reception. All new patients will be asked to make an appointment with one of the Practice Nurses. The appointment gives us the opportunity to meet you and discover your relevant medical history before you medical records become available.

Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of Race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

 

Help us to help you

We strongly support the NHS policy of zero tolerance for anyone attending the Surgery who is abusive to GP’s or staff, either verbally or physically. These patients will risk removal from the Practice List. In extreme cases, the police will be called. The patient will receive written reasons from the Practice as to the reason for removal.

Medical Reports

If you give permission for a medical report for whatever reason, the doctors are obliged to include all relevant information in your medical records. Under the access to medical reports act 1988, any patient is entitled to see the report before it is sent off. Prior notice is needed and you will be given 21 days to make any comments before the report is sent off. Under the data protection act patients are entitled to request a copy. There is a fee for this service.

Are you a carer or being cared for?

In order that the GP can do all he can to help when dealing with your needs, it is important that you inform us if you are a carer or being cared for. Please ask for a form at reception.

Repeat Prescriptions

You can order your repeat prescription by any of the following methods: –

  • By handing in your request at the reception desk.
  • By posting your request to the surgery, (please remember to enclose a S.A.E. if you want it posted back).
  • By using one of the local chemist collection services.

Tick the items you need on the right hand side of your previous prescription form. Your new prescription will be ready for collection 48 hours later. Please allow for the fact that both Surgeries are only open Monday – Friday when ordering your prescription.

Requests for repeat prescriptions are not taken over the telephone.

Repeat medication requires re-authorisation by a Doctor every 6-12 months. The back of your prescriptions will inform you of your review date. Please make an appointment.

 

Home Visits

Home visits take several times longer than a surgery consultation.

Facilities at the Surgery are far better for examination and treatment. We do understand that it is sometimes impossible for some patients to attend surgery and are happy to visit the housebound and infirm at home. If possible, please ring the Surgery before 10.30am if a home visit is needed. Please remember no transport is not a reason for a home visit. We expect patients to use public transport.

Outside Surgery Hours

For urgent appointments and home visits outside normal surgery hours please telephone the surgery number on 01495302716 and you will be given the Out of Hours Telephone Number. Aneurin Bevan University Health Board is responsible for commissioning the out of hour’s services.

Private Medical Examinations

All doctors at the surgery perform medical examinations for the special purposes e.g. Fitness to travel/pre-employment/driving medicals/insurance. A fee is payable for these medicals. Please ask reception for the current fees for work not carried out under general medical services. The fee must be paid at the time of the medical – no invoice will be sent out.

Change of address or number

If you change your address or telephone number please let reception know as soon as possible.

District Nurses

Our District Nursing Team is not based here at the surgery but can be contacted via the surgery receptionists.

Telephone advice

Patients requiring telephone advice only may speak to one of the Duty Doctors when they are not consulting.

Health Visitors

Health Visitors work at the Practice fortnightly. Health Visitors are trained Nurses who have particular skills in the field of Child Health. They will be able to help you with assessment of child development, feeding, sleep and behavior problems. The health visitor also runs weekly baby and Child Health Surveillance clinics at the Surgery. Child Health Surveillance will be shared by the Health Visitor and the Doctor.

Midwife

The Community Midwife holds weekly clinics on Thursdays at the Surgery. Your ante-natal and post-natal care will be shared by the Midwife and the Doctor.

We provide full antenatal and post-natal care

Temporary Residents

You are able to see a Doctor anywhere in the UK if you are away from home and in need of medical help. You can do this simply by asking to see a Doctor as a temporary resident and completing the necessary form. We will be happy to see any relatives or friends staying with you if they need to see a Doctor.
